Civil Rights Complaint Process:

When a Civil Rights complaint is brought to a member of nutrition services, the nutrition services team member will provide the complainant a Civil Rights Complaint Form. Paper copies of the Civil Rights Complaint Form are available in English for all team members to access in the kitchen manager's office. Civil Rights Complaint Forms are available in multiple languages on the USDA website

If the complainant cannot complete the form, a nutrition services team member will assist them, or find them appropriate assistance. Complainants with disabilities will be provided reasonable accommodations.

A Civil Rights Complaint Form returned to the district must be forwarded to the OSPI CNP within three business days.

All Civil Rights Complaints will be documented in the Civil Rights Complaint Log kept in the district administrative office. Records of complaints must be refreshed yearly, and three years plus the current year must be kept on file.
